  1. Visit the Clerk of the Board Office, Room 327, 2900 Columbus Avenue, Sandusky, Ohio - Monday through Friday, 8:00 a.m. to 4:30 p.m. or call 419-627-7675
  2. Fill out the convenient form online and e-mail to the Clerk of the Board's office
  3. Print the form available (PDF, 781KB) on line and fax it to us at 419-627-7692
