* The $25 Execution Fee is not charged when adults 16 and over with fully valid passport books apply for the passport card using a DS-82 form.

 

Passport Book By Mail - Form DS-82 (PDF, 717KB) - Passport Renewal

Receipt Within Six Weeks - These Fees Are Non-Refundable

Passport Renewal By Mail (Customer Submits)
You may qualify to use this form only if:		 Renewal Fee Execution Fee Total Fee
  1. The issue date of your last passport was less than 15 years ago;
  2. You were 16 or older when it was issued;
  3. Your name is the same or you can submit name-change documentation (marriage certificate or court order);
  4. You can submit this passport; and 
  5. It is undamaged.

The passport fees for adults eligible to apply by mail using form DS-82, "Application for a U.S. Passport by Mail", are $110 for the passport book and $30 for the passport card.

Add $60 Per Application and Two-Way Overnight Delivery

Expedite fee does not include overnight delivery.
Customer must arrange for overnight delivery to ensure receipt within two weeks.

If international departure in less than two weeks, or passport is needed for foreign visas, application at the Regional Passport Agency may be advised.  Appointment required.
